Clinical and cost effectiveness of endoscopic bipolar radiofrequency ablation for the treatment of malignant biliary obstruction: a systematic review
BACKGROUND: Early evidence suggests that using radiofrequency ablation as an adjunct to standard care
(i.e. endoscopic retrograde cholangiopancreatography with stenting) may improve outcomes in patients
with malignant biliary obstruction. OBJECTIVES: To assess the clinical effectiveness, cost-effectiveness and potential risks of endoscopic
bipolar radiofrequency ablation for malignant biliary obstruction, and the value of future research. DATA SOURCES: Seven bibliographic databases, three websites and seven trials registers were searched
from 2008 until 21 January 2021. REVIEW METHODS: The study inclusion criteria were as follows: patients with biliary obstruction caused
by any form of unresectable malignancy; the intervention was reported as an endoscopic biliary
radiofrequency ablation to ablate malignant tissue that obstructs the bile or pancreatic ducts, either to
fit a stent (primary radiofrequency ablation) or to clear an obstructed stent (secondary radiofrequency
ablation); the primary outcomes were survival, quality of life or procedure-related adverse events; and
the study design was a controlled study, an observational study or a case report. Risk of bias was
assessed using Cochrane tools. The primary analysis was meta-analysis of the hazard ratio of mortality.
Subgroup analyses were planned according to the type of probe, the type of stent (i.e. metal or plastic)
and cancer type. A de novo Markov model was developed to model cost and quality-of-life outcomes
associated with radiofrequency ablation in patients with primary advanced bile duct cancer. Insufficient
data were available for pancreatic cancer and secondary bile duct cancer. An NHS and Personal Social
Services perspective was adopted for the analysis. A probabilistic analysis was conducted to estimate
the incremental cost-effectiveness ratio for radiofrequency ablation and the probability that
radiofrequency ablation was cost-effective at different thresholds. The population expected value of
perfect information was estimated in total and for the effectiveness parameters. RESULTS: Sixty-eight studies (1742 patients) were included in the systematic review. Four studies
(336 participants) were combined in a meta-analysis, which showed that the pooled hazard ratio for
mortality following primary radiofrequency ablation compared with a stent-only control was 0.34
(95% confidence interval 0.21 to 0.55). Little evidence relating to the impact on quality of life was found. There was no evidence to suggest an increased risk of cholangitis or pancreatitis, but radiofrequency
ablation may be associated with an increase in cholecystitis. The results of the cost-effectiveness analysis
were that the costs of radiofrequency ablation was ÂŁ2659 and radiofrequency ablation produced
0.18 quality-adjusted life-years, which was more than no radiofrequency ablation on average. With an
incremental cost-effectiveness ratio of ÂŁ14,392 per quality-adjusted life-year, radiofrequency ablation
was likely to be cost-effective at a threshold of ÂŁ20,000 per quality-adjusted life-year across most
scenario analyses, with moderate uncertainty. The source of the vast majority of decision uncertainty
lay in the effect of radiofrequency ablation on stent patency. LIMITATIONS: Only 6 of 18 comparative studies contributed to the survival meta-analysis, and few data
were found concerning secondary radiofrequency ablation. The economic model and cost-effectiveness
meta-analysis required simplification because of data limitations. Inconsistencies in standard reporting
and study design were noted. CONCLUSIONS: Primary radiofrequency ablation increases survival and is likely to be cost-effective. The
evidence for the impact of secondary radiofrequency ablation on survival and of quality of life is limited.
There was a lack of robust clinical effectiveness data and, therefore, more information is needed for this
indication. FUTURE WORK: âFuture work investigating radiofrequency ablation must collect quality-of-life data. Highquality randomised controlled trials in secondary radiofrequency ablation are needed, with appropriate

Common Ground: A Place to Grow
Common Ground (CG) High School is a public charter school in New Haven, CT with approximately 200 students. CGâs mission is to cultivate habits of healthy living and sustainable environmental practices within its community. The school maintains an urban farm that provides food for students, staff, and approximately 2,500 residents of low-income neighborhoods in the area. The current literature suggests that CGâs programming should have positive health outcomes for its students and their families. CG has data on these outcomes via an online survey distributed to students, but does not yet have the internal capacity to analyze this data. This project aims to develop a protocol that enables CG faculty to autonomously analyze data and determine key impacts and outcomes. A current YSPH student-led team analyzed data from the student surveys in order to provide feedback on CGâs health-related programs and understand studentsâ health behaviors. Results from analysis were then used to conduct Community Data Discussion Groups to garner feedback from students and parents about how Common Ground has impacted their health and leadership behaviors. Detailed analysis protocols were created to enable CG faculty to independently analyze data. CG faculty was trained to implement protocol, which will allow for future independent data analysis. Overall, the results suggest that Common Groundâs programming and curriculum has had a positive impact on studentsâ health behaviors. We recommend that Common Ground expand its health-related and farm-based programming as much as possible, paying special attention to improving school lunch and allowing for physical activity. We encourage Common Ground to continue using a mixed methods approach to evaluate its programs, use the provided data analysis protocol, and work in conjunction with CARE and YSPH as they transition into independent research.https://elischolar.library.yale.edu/ysph_pbchrr/1047/thumbnail.jp

Incidence, Reproductive Outcome, and Economic Impact of Reciprocal Translocations in the Domestic Pig
Pigs (Sus scrofa) have vast economic importance, with pork accounting for over 30% of the global meat consumption. Chromosomal abnormalities, and in particular reciprocal translocations (RTs), are an important cause of hypoprolificacy (litter size reduction) in pigs. However, these do not necessarily present with a recognizable phenotype and may cause significant economic losses for breeders when undetected. Here, we present a reappraisal of the incidence of RTs across several European pig herds, using contemporary methodology, as well as an analysis modelling the economic impact of these abnormalities. Molecular cytogenetic investigation was completed by karyotyping and/or multiprobe FISH (fluorescence in situ hybridisation) between 2016â2021, testing 2673 animals. We identified 19 types of chromosome abnormalities, the prevalence of these errors in the database was 9.1%, and the estimated incidence of de novo errors was 0.90%. Financial modelling across different scenarios revealed the potential economic impact of an undetected RT, ranging from ÂŁ69,802 for an individual affected terminal boar in a commercial farm selling weaned pigs, to ÂŁ51,215,378 for a genetics company with an undetected RT in a dam line boar used in a nucleus farm. Moreover, the added benefits of screening by FISH instead of karyotyping were estimated, providing a strong case for proactive screening by this approach
Smoke, curtains and mirrors: the production of race through time and title registration
This article analyses the temporal effects of title registration and their relationship to race. It traces the move away from the retrospection of pre-registry common law conveyancing and toward the dynamic, future-oriented Torrens title registration system. The Torrens system, developed in early colonial Australia, enabled the production of âcleanâ, fresh titles that were independent of their predecessors. Through a process praised by legal commentators for âcuringâ titles of their pasts, this system produces indefeasible titles behind its distinctive âcurtainâ and âmirrorâ, which function similarly to magiciansâ smoke and mirrors by blocking particular realities from view. In the case of title registries, those realities are particular histories of and relationships with land, which will not be protected by property law and are thus made precarious. Building on interdisciplinary work which theorises time as a social tool, I argue that Torrens title registration produces a temporal order which enables land market coordination by rendering some relationships with land temporary and making others indefeasible. This ordering of relationships with land in turn has consequences for the human subjects who have those relationships, cutting futures short for some and guaranteeing permanence to others. An international recommendation framework of core competencies in health informatics for nurses.
Background: While health informatics recommendations on competencies and education serve as highly desirable corridors for designing curricula and courses, they cannot show how the content should be situated in a specific and local context. Therefore, global and local perspectives need to be reconciled in a common framework.Objectives: The primary aim of this study is therefore to empirically define and validate a framework of globally accepted core competency areas in health informatics and to enrich this framework with exemplar information derived from local educational settings.Methods: To this end, (i) a survey was deployed and yielded insights from 43 nursing experts from 21 countries worldwide to measure the relevance of the core competency areas, (ii) a workshop at the International Nursing Informatics Conference (NI2016) held in June 2016 to provide information about the validation and clustering of these areas and (iii) exemplar case studies were compiled to match these findings with the practice. The survey was designed based on a comprehensive compilation of competencies from the international literature in medical and health informatics.Results: The resulting recommendation framework consists of 24 core competency areas in health informatics defined for five major nursing roles. These areas were clustered in the domains âdata, information, knowledgeâ, âinformation exchange and information sharingâ, âethical and legal issuesâ, âsystems life cycle managementâ, âmanagementâ and âbiostatistics and medical technologyâ, all of which showed high reliability values. The core competency areas were ranked by relevance and validated by a different group of experts. Exemplar case studies from Brazil, Germany, New Zealand, Taiwan/China, United Kingdom (Scotland) and the United States of America expanded on the competencies described in the core competency areas.Conclusions: This international recommendation framework for competencies in health informatics directed at nurses provides a grid of knowledge for teachers and learner alike that is instantiated with knowledge about informatics competencies, professional roles, priorities and practical, local experience. It also provides a methodology for developing frameworks for other professions/disciplines. Finally, this framework lays the foundation of cross-country learning in health informatics education for nurses and other health professionals
Climate change effects on phytoplankton depend on cell size and food web structure
We investigated the effects of warming on a natural phytoplankton community from the Baltic Sea, based on six mesocosm experiments conducted 2005â2009. We focused on differences in the dynamics of three phytoplankton size groups which are grazed to a variable extent by different zooplankton groups. While small-sized algae were mostly grazer-controlled, light and nutrient availability largely determined the growth of medium- and large-sized algae. Thus, the latter groups dominated at increased light levels. Warming increased mesozooplankton grazing on medium-sized algae, reducing their biomass. The biomass of small-sized algae was not affected by temperature, probably due to an interplay between indirect effects spreading through the food web. Thus, under the higher temperature and lower light levels anticipated for the next decades in the southern Baltic Sea, a higher share of smaller phytoplankton is expected. We conclude that considering the size structure of the phytoplankton community strongly improves the reliability of projections of climate change effects
Grazer diversity effects in an eelgrassâepiphyteâmicrophytobenthos system
The dramatic loss of biodiversity and its consequences for ecosystem processes have been of considerable interest in recent ecological studies. However, the complex and interacting processes influencing diversity effects in multitrophic systems are still poorly understood. We used an experimental eelgrass system to study the effects of changing richness of three consumer species on the biomass, diversity and taxonomic composition of both epiphytic and benthic microalgal assemblages. After 1Â week, consumer richness enhanced the grazing impact on epiphyte biomass relative to single consumer treatments and a positive effect of consumer richness on prey diversity was found. Moreover, strong effects of consumer species identity on taxonomic composition were found in both microalgal assemblages. However, the effects of consumer richness were not consistent over time. The consequences of high nutrient availability seemed to have masked consumer richness effects
